This form needs to be 100% accurate and have all request noted in the highlights section. - Start Date should be on or very quickly after the Order Start Date listed.
- All RTO should be listed on the cover page, must be less than 7 days.
- RTO will not be approved in the first week of assignment or holidays.
- All facilities the traveler is open to working in should be listed on the submission.
- If interested in any location, please include what states (I.E. Any AZ, Any CO, Any state, etc.)
- All shifts the traveler is open to working on should be listed on the submission.
- All departments the traveler is open to working must be checked on the cover page.
- All candidates submitted should be preclosed and ready to accept.
- Please let us know if the traveler withdraws or is no longer available.
